General health status, symptom survey and lifestyle appraisal questionnaires      
Metabolic In-Office Assessments:
Body composition BIA testing: lean body mass, body fat %, total body water distribution
Body type circumferences, skin folds, BMR, height and body weight measurements    
Review of general medical history, family genetics and history of weight changes
Review of present food intake eating plan, exercises, sleep pattern and daily lifestyle
Review and examination of metabolic balance and health status of body systems
Stress exposure assessment and environmental chemical exposures

Second Visit Consultation – Recommendations are provided for:
Personalized treatment plan addressing the factors causing metabolic and weight imbalances
Individualized metabolic balanced whole foods eating plan
Time efficient balanced exercise plan and health promoting lifestyle practices
Nutritional supplementation to support energy, control appetite and metabolic function

This initial evaluation and assessment is the most important and also the most time intensive step in designing your program.  It requires you to complete a detailed health appraisal questionnaire, in-office testing of body composition and a detailed examination with the doctor.  It will typically take at least 60 to 90 minutes scheduled as two separate appointment times.

STAYING ON TRACK
WEEKLY & MONTHLY CONSULTATIONS PROGRAM

Weekly in-office 15-20 minute consultation appointments (or by telephone if necessary)
Review of progress and weekly food, lifestyle and exercise journal
Goal setting, continued education, homework and treatment plan modifications
Report of findings on laboratory testing (may need additional consultation time)
